Box 2253, Fallbrook, CA 92088 (Phone number 760-723-0464.  Email:  paul0819@aol.com) This agreement is made and entered into this ______ day of ________, 20___, by and between Strings For Your Heart, hereinafter referred to as “SFYH” and ____________________________, hereinafter referred to as “Client.”   SFYH agrees to perform at (place) ____________________________________________ on (date) __________________________.   Arrival time shall be ___________________.  Start time shall be __________________.  Finish time shall be ________________.  This will comprise _______ sets of________ minutes’ approximate duration, divided by ____ breaks of ______ minutes’ approximate duration.  SFYH shall provide ______ (number) of player/players in the form of a String ___________________. SFYH cannot perform in direct sunlight or extreme weather conditions due to the sensitivity of the instruments.  Therefore, outdoor performances must include some provision for shelter/shade for SFYH.  SFYH will be given access to bathrooms and break space.  SFYH will be provided with adequate parking with loading space for equipment transfer before and after the performance.  SFYH will be provided a secure and safe place in which to store instruments and gear when not in use.   Neither Client nor any agent on Client’s behalf will take photographs, video, audio recordings, or motion pictures of SFYH’s performance under this agreement without obtaining SFYH’s prior written approval or without paying SFYH any compensation for such activities.  This excludes wedding photos and wedding videos for private viewing.   Either Client or SFYH may terminate this agreement for any reason whatsoever and without any obligation to the other if the other party receives written notice of the party’s desire to terminate this agreement within sixty (60) days in advance of the date of SFYH’s performance. Client will protect, defend, indemnity, and save harmless SFYH, its members, and agents from and against any and all liability, loss, damage, expenses, costs (including without limitation costs of the fees of litigation of every nature) arising out of or in connection with SFYH’s performance of this agreement or its failure to comply with any of its obligations contained in this agreement.   Client agrees to pay SFYH at fee of $___________.  A non-refundable deposit of $75.00 is due at the signing of this agreement.  A balance of $_______________ is due 14 days prior to the date of performance.  The above fees are exclusive of the customary gratuities.  Overtime will be billed at the rate of $100.00 per hour-half, due and payable within two (2) weeks of completion of the performance.  All checks must be made payable to Strings For Your Heart. An additional fee of $50.00 will be charged, which is due at the time the balance is paid, for any venues outside of Orange County, California. SFYH is booked as an acoustic group.  No electrical equipment will be provided with the exception of music stand lights if necessary.
